Individual Development Accounts (VIDA) Program Intermediaries Corner
VIDA intermediaries are responsible for marketing the VIDA program, providing technical assistance to applicants and enrolled participants, and providing the financial and asset-specific training to participants.
The forms and information listed below are for existing VIDA intermediaries.
Information for organizations interested in becoming an intermediary is available online. Several of the below forms have been revised as indicated by the “2007” date on the document link. Please use these revised materials and discard previous versions.

Saver Forms
Income Limits - Use this form as a guide to determine income eligibility of prospective savers for the VIDA program.
Intermediary’s Applicant Checklist - Use this form as a cover page to communicate information about the candidate to the VIDA application review committee and to ensure all required documents are included in the application package.
Candidate Application - When your client is ready to take advantage of the program work with them to complete the application form and then review their answers for clarity and completeness.
Wachovia Application to open a VIDA Account - Savers wanting to bank at Wachovia must complete this form. Those selecting VIDA accounts through BB&T do not need to send in this form at this time.
Saver Agreement Between DHCD and Participant - This form spells out the responsibilities of the agency and the saver as it relates to their participation in the program. There are two versions of the saver agreement, one for savers selecting a VIDA account through Wachovia and one for savers selecting a VIDA account through BB&T.
Qualified Withdrawal Form - The saver is ready to purchase their asset! Use this form to request a match payment request from DHCD. Back-up documentation is required for each payment. Examples of acceptable documentation can be found in the Administrative Manual. To expedite the process you may fax the form to DHCD at (804) 371-7093; however, you must follow-up by immediately contacting a VIDA program staff member via email or telephone at (804) 371-7030 and allow five business days from the date the request was submitted for DHCD to process your request (note this does not include mail time). Please plan accordingly.
Non-qualified Withdrawal Form - Use this form if the saver has an emergency withdrawal and needs access to their VIDA savings. Emergencies hinder participants from reaching their savings goals, so please try to exhaust other resources before tapping into the VIDA savings account. If a withdrawal is necessary, to expedite the process you may fax the form to DHCD at (804) 371-7093. You must follow-up by immediately contacting a VIDA program staff member via email or telephone at (804) 371-7030 and allow five business days from the date the request was submitted for DHCD to process your request (note this does not include mail time). Please plan accordingly.
Program Implementation Information
Administrative Manual - This document outlines the program, roles and responsibilities, qualification criteria, and key terms.
Monthly Report Form - Use this form to report your organization’s monthly program activity related to the delivery of VIDA services. The monthly report is due by the tenth of the following month.
Certification of Signatures - This document identifies which individuals within your organization may sign request for reimbursements (or draw requests) to our agency. It is completed when you first become an intermediary and if your signatories should change, please submit an updated form to DHCD.
Form W-9 - This form is needed to allow your organization to receive administrative payments.
Draw Request Form/Accounting Voucher - Official form to request reimbursement from DHCD for an intermediary’s administrative fees. Refer to the 2007 Administration Manual for a list of allowable administrative costs.
